First Visit
When you arrive please bring your driver’s license, insurance card (if applicable) and any diagnostic test results (MRI, CAT scan, previous X-Rays) that may help better assess your condition. We will verify your insurance benefits for your convenience as some health insurance benefits cover therapeutic massage in a clinical facility.
Please arrive 15 minutes before your appointed time to complete an intake form or click on our “Patient Forms” tab to print and fill out at your convenience before your appointed time. The therapist will consult with you for about 5-10 minutes during your session to address your health concerns after reviewing your entrance form. You will be given a gown to prepare for your massage. Depending on your massage session, an extended session will be 1 ½ hours in duration with 80mins of hands on time and 50 minutes hands on time with an hour session.
